Ideally I would like clicking on the picture to go to my blog, but I assume you will want a click on the image to go to Dr Ellis's site - so in that case just include a link in writing (I don't care what you write, "Image Source" is fine) adjacient to the image indicating its source.
4) You can only use the image on the web site you described in your comment. If you want to use it for another purpose please seek permission from me first.

I recieved your comment but all my attempts to e-mail you have been rejected by your mail server.
Here is what I tried to send in the e-mail:
Hi Professeur Giroux,
I would be pleased for you to use the image (IMGD5593) for the web site you describe under the following conditions:
1) You resize the image so that the largest dimension is 400 pixels or less.
2) The copyright notice is not cropped off the image (eventhough it will be resized along with the rest of the image).
3) You include a link adjacient to the picture that links back to the original blog post:
